Tier 1

$ 650 ($300 average savings)

Year
  • Meant for patients who prioritize preventive care and don’t have active periodontal disease.
  • Includes 2 hygiene visits with laser therapy!
  • Patients will receive 10% savings on most restorative treatments.
  • Additional benefits include covered diagnostic imaging, a comprehensive oral cancer screening, one emergency/limited exam, and preferred pricing on custom nightguards & retainers.
  • No deductibles, no waiting periods, no annual maximums, no claims forms, and predictable, locked-in pricing!

Tier 2

$ 1,000 ($525 average savings)

Year
  • Meant for patients with active periodontal disease/requiring periodontal maintenance.
  • Includes 3-4 covered visits each year (frequency is determined by your clinical needs).
  • 15% discount on all restorative dentistry needs is built into the plan.
  • Patients will also receive two routine examinations, one emergency examination, diagnostic imaging as needed, fluoride therapy when indicated, and other non-surgical periodontal therapies.

Tier 3

  • Meant for patients with dental implants, complex restorative needs, or those who would like concierge-level care.
  • Includes 4 covered hygiene visits with implant-specific protocols, 1 comprehensive exam, and 1 routine exam.
  • Unlimited emergency examinations.
